Appointments must be cancelled 24 hours before the appointment to avoid cancellation fee.

Any appointment cancelled less than 24 hours will result in full service cost charge. Deposit will also be forfeited.

NO CALL/NO SHOW is required to pay 50% of the next session and will be charged full price of missed appointment. Deposit will be forfeited.

Appointments will be canceled after 15 minutes if the client has not shown up.

I am not authorized to prescribe medications or diagnose any client.

All clients information is confidential unless there an informed consent signed

There is a draping procedure that is to be followed.

Plastic Surgery appointments can be booked up "Post Operative Treatments."

Deposits are NON-REFUNDABLE.

Gift cards can be used to pay the deposit. For the expiration date put the month purchased and the following year. If you have purchased gift cards from Square, please call to set up an appointment. Gift cards must be valid.

Paying the deposit is an agreement that the policy has been read and understood.
